Foundations of Hope

Foundations of Hope is the front door to our care programs. A person in crisis meets one case manager, who looks at the whole situation and builds a plan with them.

What we do

  • A first conversation about home, work, money, health, family and faith.
  • A written plan with goals the person helps set.
  • Connections to our other care programs and to outside services.
  • Regular check-ins until life is steady again.

Why we do it

People in crisis rarely face one problem. Rent, work, health and family pull on each other, and a single referral seldom holds. One person walking alongside, with one plan, gives help a chance to last.

How we do it

  1. First conversation

    A case manager listens and asks about each part of life. Questions about faith are optional.

  2. A plan

    Together you set goals and milestones and list the help needed to reach them.

  3. Connections

    The case manager connects you with housing, work, family or mental health support, at CIAN or with other services.

  4. Check-ins

    You meet regularly to review progress and adjust the plan. Milestones are celebrated.
